The Caregiver Resource Center Offers "Aging Lifecare Planning"
No one looks forward to an unexpected personal or medical situation, that is complex, overwhelming and catches you off guard.
As we age, we will all need to explore our current, as well as our evolving needs - the “what-ifs”.
If left unaddressed, these situations have the potential to impact on our health, safety, independence, and quality of life.
Aging Lifecare Planning
“Aging Lifecare Planning” starts with a comprehensive assessment to identify a person’s abilities, needs, wishes and goals.
This includes a person’s health and mental health, financial resources, residential preferences, support network, transportation, and social and emotional well being.
From there our aging lifecare advocate helps a person design a customized plan, to address each aspect of their current and evolving needs as they age.
